In the course of all the hard work each of you does on Champions projects, you may need one of the forms below
The Champions Donor Solicitation Letter is for the 2020-2021 School Year. It is for soliciting donations from community partners.
Most vendors will accept the Tax Exempt form and remove sales tax charges from purchases you make for Champions projects.
Complete the Reimbursement Form and put it in the Champions box in the office when Champions owes you money for a purchase you made personally for a Champions project. Currently available in the office.
Complete the Check Request Form when you need Champions to write a check to a vendor for a Champions project, for example to make a deposit to hold an event venue, or for a large purchase of supplies you are ordering for a project. Currently available in the office.
